Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.

Here's the concern with pairing red yeast rice and selpercatinib (Retevmo). Both can be tough on the liver. Selpercatinib is a cancer medicine known to sometimes raise liver enzymes, and red yeast rice naturally contains lovastatin, a statin that can do the same thing. Stacking two liver-stressing products together could, in theory, add up and make liver irritation more likely.

I want to be honest that this is a theoretical caution based on how these products behave, not a proven, common problem. Still, because your cancer team likely already checks your liver bloodwork on selpercatinib, it's worth a quick chat with them before adding red yeast rice. Please don't start or stop anything on your own.

Here's the key thing to know about pairing red yeast rice with Retevmo (selpercatinib). Red yeast rice naturally contains lovastatin, a statin that your liver clears using an enzyme called CYP3A4. Selpercatinib can mildly slow that same enzyme down. In theory, that means the statin from red yeast rice could build up a bit higher than usual in your body.

Why does that matter? More statin in your system raises the odds of statin-type side effects, mainly muscle aches, weakness, and rarely liver irritation. I want to be honest though: this concern is theoretical, based on how the drugs work rather than real cases, and red yeast rice products vary a lot in strength. Since you're already managing cancer treatment, please loop in your oncology team or pharmacist before adding it.

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
