Theoretically, concomitant use might increase the risk of liver damage.
Red yeast rice contains varying levels of the drug lovastatin. Lovastatin can cause liver damage in some people. Some clinical research suggests that supplements containing red yeast rice might increase liver enzyme levels in some, but not all, participants. Cases of acute hepatitis have been associated with red yeast rice. Combining it with hepatotoxic drugs might further increase this risk.

Here's the concern with pairing red yeast rice and Zileuton (Zyflo): both can be hard on your liver. Zileuton already carries a known risk of raising liver enzymes, which is why your doctor usually checks bloodwork when you start it. Red yeast rice naturally contains lovastatin (a statin), and statins can also stress the liver. Stacking two liver-taxing products together may add up.

To be fair, this is a theoretical caution based on how these substances behave, not a documented disaster, so I don't want to alarm you. But watch for warning signs like unusual fatigue, belly pain on your upper right side, dark urine, or yellowing of your skin or eyes. Please check with your pharmacist or doctor before combining them, especially since red yeast rice doses vary a lot between brands.

Here's the connection worth knowing. The cholesterol-lowering ingredient in red yeast rice is really lovastatin, a statin your liver clears using an enzyme called CYP3A4. Zileuton can slow down that same enzyme, so taking the two together could let lovastatin build up higher than expected in your blood.

When statin levels run high, the main worry is muscle trouble: aches, soreness, weakness, and in rare cases more serious muscle breakdown that can affect the kidneys. This caution is theoretical, based on how these substances behave rather than reports of harm, and red yeast rice products vary a lot in strength. Still, since both touch CYP3A4, please check with your pharmacist or doctor before combining them.

What Is Zileuton Used For?

Brand names: Zyflo, Zyflo CR

Zileuton is used for the ongoing prevention and treatment of asthma in adults and adolescents aged 12 and older. It is a maintenance medicine — meaning it is taken regularly to keep asthma under control, not to stop an asthma attack once it has started.

Zyflo (the tablet form) and zileuton extended-release tablets are both approved for this same purpose. Neither form should be used to treat a sudden asthma attack. However, both can be continued during a flare-up of asthma while you also use your rescue inhaler.
