Here's the simple version: your Xelstrym patch already revs up your nervous system to help with focus and attention. Securinega suffruticosa contains an alkaloid called securinine that also stimulates the nervous system, mainly by blocking the brain's calming (GABA) signals. Put the two together and you could be stacking one stimulant on top of another.

The concern is that combined stimulation might push things too far, meaning a faster or pounding heartbeat, higher blood pressure, jitteriness, trouble sleeping, anxiety, or, in a worst case, seizures. I'll be honest that this is a theoretical worry based on how securinine behaves in animal studies, not proven cases in people. Still, this plant isn't a safe self-treatment, so please run it by your doctor or pharmacist before mixing anything with your patch.
