Why this applies to Tigecycline

Tigecycline falls under AMINOGLYCOSIDE ANTIBIOTICS — the group Sodium Bicarbonate interacts with.

Theoretically, sodium bicarbonate may increase the risk for hypokalemia in patients receiving aminoglycosides.
Orally, use of excessive sodium bicarbonate (such as the intake of "tablespoons" of sodium bicarbonate daily or up to one box of baking soda weekly) has been associated with cases of hypokalemia. Furthermore, when administered intravenously, the most common complication of sodium bicarbonate is hypokalemia. Nephrotoxicity caused by aminoglycosides may lead to increased urinary losses of various electrolytes, including potassium.

Here's the honest picture for tigecycline (Tygacil). The interaction we have on file is grouped under aminoglycoside antibiotics, and it's really about your potassium level, not how the drug itself is broken down. The concern is that large amounts of sodium bicarbonate can lower potassium (hypokalemia), and certain antibiotics that stress the kidneys can do the same. Tigecycline is given as an IV in the hospital, so your team is already watching your labs closely.

A couple of things ease my mind: this is a theoretical concern based on pharmacology, and it really only applies to heavy baking soda use (think tablespoons a day). A normal antacid pinch now and then is unlikely to matter. Still, since you're on an IV antibiotic, just mention any baking soda use to your pharmacist or doctor.

What Is Tigecycline Injection Used For?

Brand name: Tygacil

Tygacil (tigecycline for injection) is approved for adults 18 and older to treat three types of serious bacterial infections: complicated skin and skin structure infections, complicated intra-abdominal (stomach and intestinal) infections, and community-acquired bacterial pneumonia (pneumonia caught outside a hospital).

It is not approved for diabetic foot infections or for pneumonia caught in a hospital setting, including ventilator-associated pneumonia — clinical trials showed it did not work well enough for those uses and was associated with higher mortality in patients with ventilator-associated pneumonia.
