Why this applies to Entrectinib

Entrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Your body relies heavily on an enzyme called CYP3A4 to clear entrectinib from your system. In lab and animal studies, sulforaphane appears to slow that enzyme down. If that same effect happened in people, entrectinib could build up to higher levels than intended, which might mean more of its typical side effects like dizziness, fatigue, constipation, swelling, taste changes, or heart rhythm changes (a longer QT interval).

Here's the reassuring part: this has only been seen in test tubes and animals, never confirmed in humans, so it stays theoretical. The amount of sulforaphane in a normal serving of broccoli is very unlikely to matter. A concentrated supplement is the bigger question mark. Since entrectinib is a serious cancer medicine, please run any supplement past your oncologist or pharmacist first.

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC) — a type of lung cancer where the ROS1 gene is rearranged — as confirmed by an FDA-approved test.

Rozlytrek is also approved for adult and pediatric patients older than 1 month with solid tumors that have an NTRK gene fusion (a specific genetic alteration), detected by an FDA-approved test, that have spread or cannot be removed without causing serious harm, and have worsened after prior treatment or have no good treatment alternatives. This NTRK indication was granted accelerated approval based on tumor response rates; continued approval may depend on results from confirmatory trials.
