Hydromorphone falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Here's the big concern with this pairing: hydromorphone is already a strong opioid that slows your breathing and makes you sleepy. Tianeptine, especially in the high doses found in 'gas station' products, behaves like an opioid too. Stacking the two means their sedating effects can pile on top of each other, so you may get dangerously slow or shallow breathing, heavy drowsiness, confusion, or even loss of consciousness. That same kind of combination (high-dose tianeptine plus another depressant) put one man in the hospital with breathing failure and pneumonia.

This warning is based on how these drugs act rather than large studies, but with a potent opioid like hydromorphone I wouldn't take chances. Please talk with your doctor or pharmacist before mixing anything with your hydromorphone.

What Is Hydromorphone Used For?

Brand names: Dilaudid, Hydromorph Contin, Palladone

Hydromorphone is used to manage pain severe enough to require an opioid analgesic — when non-opioid medicines or other options have not worked or are not expected to work well enough. It is available in several forms: oral tablets, oral solution, rectal suppositories, and injectable formulations given into a vein, muscle, or under the skin.

Hydromorphone hydrochloride extended-release tablets are specifically for severe, persistent pain that requires around-the-clock opioid treatment for an extended period, and are only for opioid-tolerant patients — they are not for as-needed use. The high-potency injectable formulation is also reserved for opioid-tolerant patients who need higher opioid doses.
