Topiramate falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Here's the thing to know about pairing these two: tianeptine, especially at the high doses found in those gas station products, can act a lot like an opioid and pile on sedation. Topiramate isn't an opioid, but it can already cause drowsiness, dizziness, slowed thinking, trouble concentrating, and clumsiness for some folks. Stack tianeptine on top, and those foggy, sleepy effects can add up.

This concern is based on how the drugs behave (pharmacology) and a case where high-dose tianeptine plus a sedative led to dangerously slow breathing, so I'd take it seriously even though it isn't proven for topiramate specifically. Honestly, I'd steer clear of unregulated tianeptine altogether. Please loop in your doctor or pharmacist before combining anything.

What Is Topiramate Used For?

Brand names: Topamax, Trokendi XR, Qudexy XR, Eprontia

  • Topiramate is used alone or with other medications to treat certain types of seizures including primary generalized tonic-clonic seizures (formerly known as a grand mal seizure; seizure that involves the entire body) and partial onset seizures (seizures that involve only one part of the brain). Topiramate is also used with other medications to control seizures in people who have Lennox-Gastaut syndrome (a disorder that causes seizures and developmental delays). Topiramate is also used to prevent migraine headaches but not to relieve the pain of migraine headaches when they occur. Topiramate is in a class of medications called anticonvulsants. It works by decreasing abnormal excitement in the brain.
