Propoxyphene falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Here's the simple version: propoxyphene is an opioid pain reliever, so it already slows down your breathing and makes you drowsy. Tianeptine, especially in the higher doses found in those over-the-counter 'gas station' products, can act a lot like an opioid too. Stack the two together and those effects can pile on top of each other. That means more sedation, slowed breathing, confusion, and a real risk of overdose.

I'll be honest, this concern comes from pharmacology and a case report rather than big studies, so it's a theoretical warning. But the combo of two opioid-acting substances is one I take seriously. Please don't start or change anything on your own. Talk with your doctor or pharmacist first.

Brand names: Darvon, Prophene-65, Margesic, Davon

This three-ingredient combination — butalbital, acetaminophen, and caffeine — is used to relieve the symptoms of tension headache (also called muscle contraction headache). All supported products, including tablets, capsules, and oral solution, share this same approved use.

It is worth knowing that evidence for using this combination to treat repeated, frequent tension headaches over time is limited. Because butalbital can be habit-forming, extended or repeated use is not recommended.
