Gabapentin falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Here's the simple version: both gabapentin and tianeptine can slow down your central nervous system. Gabapentin already tends to cause drowsiness, dizziness, and slowed breathing in some people, and tianeptine (especially at the high doses found in those gas station products) acts a lot like an opioid. Stack them together and those calming effects can pile up, leaving you overly sedated, foggy, unsteady on your feet, or in the worst case breathing too slowly.

This concern is based on how these drugs behave rather than big studies, so it's more of a theoretical caution. Still, there's a real case of someone running into serious breathing trouble mixing high-dose tianeptine with another sedative. I'd strongly encourage you to talk with your doctor or pharmacist before combining these.

About Gabapentin

What Is Gabapentin Used For?

Brand names: Neurontin, Gralise

Gabapentin (brands include Neurontin, Relgaabi, Gabarone, Gralise, and Horizant) is primarily used to manage postherpetic neuralgia (PHN) — the nerve pain that lingers after a shingles outbreak — in adults. Neurontin, Relgaabi, Gabarone, and gabapentin capsules, tablets, and oral solution are also used as add-on therapy for partial onset seizures (a type of epilepsy) in adults and children aged 3 and older.

Horizant (gabapentin enacarbil extended-release tablet) has two specific approvals: treatment of moderate-to-severe primary restless legs syndrome (RLS) in adults, and management of postherpetic neuralgia in adults. Horizant is not recommended for people who need to sleep during the day and stay awake at night. Gralise is approved only for postherpetic neuralgia and cannot be substituted for other gabapentin products due to its different release profile.
