Zonisamide falls under CNS DEPRESSANTS — the group Tianeptine interacts with.

Tianeptine has sedative effects; high doses may have additive effects with other CNS depressants.
Higher doses of tianeptine have opioid like effects and may have additive effects with other sedative drugs. In one case report, a 42-year-old male presented with respiratory depression and aspiration pneumonia due to concomitant use of high-dose tianeptine and alprazolam.

Here's the simple version: both of these can slow you down. Zonisamide, your seizure medicine, often causes drowsiness, dizziness, trouble concentrating, and slowed thinking on its own. Tianeptine can also be sedating, and at the high doses found in many gas station products it acts like an opioid, which can dangerously slow breathing. Stack the two and those calming effects can pile up.

To be fair, this concern comes from pharmacology and a case report involving high-dose tianeptine, not from solid studies on zonisamide specifically, so the risk is real but theoretical. Even so, I'd steer clear of these illegal tianeptine products entirely. Please talk with your doctor or pharmacist before mixing anything new with your seizure medication.

What Is Zonisamide Used For?

Brand names: Zonegran, Zonisade

Zonisamide is used as add-on therapy — meaning it is combined with other seizure medications, not used alone — to treat partial seizures (seizures that start in one part of the brain) in adults with epilepsy. Zonegran (capsule) and generic zonisamide capsules are approved for adults with epilepsy.

Zonisade (oral suspension) is approved as add-on therapy for partial-onset seizures in adults and in patients 16 years of age and older.
