CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Nintedanib Esylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Turmeric interacts with.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

Here's the gist with Ofev (nintedanib) and turmeric. Your liver uses an enzyme called CYP3A4 to help clear nintedanib. Turmeric's curcumin may slow that enzyme down, and if it does, more drug could linger in your system. With nintedanib, the side effects people notice most are diarrhea, nausea, vomiting, belly pain, and changes in liver tests, so higher levels could make those rougher.
The honest truth is that this concern comes mostly from lab and animal studies, not from solid human data, so it's a theoretical worry rather than a proven one. A sprinkle of turmeric in cooking is unlikely to be the issue. Concentrated curcumin capsules are where I'd pause. Please check with your pharmacist or doctor before adding a turmeric supplement.

ANTICOAGULANT/ANTIPLATELET DRUGS

Curcumin, a constituent of turmeric, has demonstrated antiplatelet effects in vitro. Furthermore, two case reports have found that taking turmeric along with warfarin or fluindione was associated with an increased international normalized ratio (INR). However, one clinical study in healthy volunteers shows that taking curcumin 500 mg daily for 3 weeks, alone or with aspirin 100 mg, does not increase antiplatelet effects or bleeding risk. It is possible that the dose of turmeric used in this study was too low to produce a notable effect.

Here's the thing to know about Ofev (nintedanib): on its own, it already carries a bleeding risk because of how it works on blood vessels. Turmeric, especially in concentrated supplement form, may mildly thin the blood by making platelets a little less sticky. Stack the two together and you could, in theory, see a bit more bruising, nosebleeds, bleeding gums, or blood that takes longer to stop.
That said, the evidence here is mixed. A couple of case reports raised a flag, but a small study didn't find much effect at lower doses. The curry-amount of turmeric you sprinkle on food is very unlikely to be a problem. Concentrated capsules are the bigger question. Before adding a turmeric supplement, please run it by your pharmacist or doctor first.

P-GLYCOPROTEIN SUBSTRATES

In vitro and animal research shows that curcuminoids and other constituents found in turmeric can inhibit P-glycoprotein expression and activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Ofev (nintedanib): it relies on a cellular pump called P-glycoprotein to help move it in and out of your body. Lab and animal studies suggest the curcumin in turmeric can slow that pump down. If that pump is blocked, a bit more nintedanib could get absorbed, which in theory might nudge up its common side effects, things like diarrhea, nausea, stomach upset, and changes in your liver tests.
The good news is this is a minor, theoretical concern based only on lab work, not real patient cases. Everyday turmeric in your cooking is very unlikely to matter. Concentrated curcumin supplements are where I'd pause. Since Ofev already tends to cause GI upset, it's worth a quick chat with your pharmacist or doctor before adding a turmeric supplement.

What Is Nintedanib Used For?
Brand name: Ofev
Nintedanib capsules are approved for adults with idiopathic pulmonary fibrosis (IPF) — a chronic, progressive scarring of the lungs — and for adults with chronic fibrosing interstitial lung diseases (ILDs) with a progressive phenotype, meaning fibrosing lung diseases that continue to worsen over time despite treatment.
Nintedanib capsules (including Ofev) are also indicated to slow the rate of decline in lung function in adults with systemic sclerosis-associated interstitial lung disease (SSc-ILD), a lung complication of the autoimmune condition systemic sclerosis (scleroderma). Note that the Nintedanib esylate capsule product is approved for IPF and chronic fibrosing ILDs with a progressive phenotype, but SSc-ILD approval is carried by Ofev and other nintedanib capsule products — always confirm with your prescriber which product you have been prescribed.
