Topotecan falls under TOPOISOMERASE I INHIBITORS — the group Turmeric interacts with.

Turmeric has antioxidant effects. There is some concern that this may reduce the activity of chemotherapy drugs that generate free radicals. However, research is conflicting.
In vitro research shows that curcumin, a constituent of turmeric, inhibits camptothecin-induced apoptosis of breast cancer cells by up to 71%. However, other in vitro research shows that curcumin augments the cytotoxic effects of camptothecin. Reasons for the discrepancies may relate to the dose of curcumin and the chemotherapeutic agents. Lower doses of curcumin might have antioxidant effects while higher doses might have pro-oxidant effects. More evidence is needed to determine what effect, if any, turmeric might have.

Here's the thinking behind this one. Topotecan (Hycamtin) is a chemotherapy drug that kills cancer cells partly by creating reactive, free-radical type damage inside them. Turmeric's main compound, curcumin, can act as an antioxidant, so the worry is that it might mop up some of that damage and blunt how well your chemo works.

I want to be honest about the strength here: this is mostly test-tube research, and it's genuinely mixed. Some lab studies show curcumin weakens drugs like topotecan, while others show it actually boosts them, possibly depending on dose. We just don't have solid human data. Because topotecan is such a high-stakes cancer treatment, I wouldn't gamble on it. Normal turmeric in your cooking is very unlikely to matter, but please clear any concentrated curcumin supplements with your oncologist or pharmacist first.

What Is Topotecan Used For?

Brand name: Hycamtin

Topotecan treats several serious cancers. The intravenous (IV) forms — sold under the brand names Hycamtin, Topotecan, and Topotecan Hydrochloride — are used for metastatic ovarian cancer that has progressed after prior chemotherapy, for small cell lung cancer (SCLC) that came back at least 60 days after first-line treatment, and for Stage IV-B, recurrent, or persistent cervical cancer (in combination with the chemotherapy drug cisplatin).

Hycamtin capsules (taken by mouth) are approved specifically for relapsed small cell lung cancer in patients who had at least a partial response to prior treatment and who are at least 45 days out from their first-line chemotherapy.
