Perampanel falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Vinpocetine interacts with.

Theoretically, vinpocetine might increase levels of drugs metabolized by CYP2C9.
In vitro research shows that vinpocetine weakly inhibits CYP2C9. However, this effect has not been reported in humans.

Here's the simple version: your seizure medicine Perampanel (Fycompa) is partly cleared by a liver enzyme called CYP2C9. In a test-tube study, vinpocetine appeared to weakly slow that same enzyme down. If that happened in your body, perampanel could clear a little more slowly and build up, which might mean stronger effects or more side effects like dizziness, sleepiness, trouble with balance, or mood and behavior changes.

The reassuring part is that this is only a theoretical, lab-based concern. It has never actually been seen in people, and the effect looked weak. So I wouldn't panic, but I also wouldn't ignore it. Please run it by your pharmacist or doctor before adding vinpocetine, and never adjust your perampanel on your own.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
