Creatine monohydrate is generally well tolerated in healthy adults but warrants caution if you have kidney problems; it's avoided in pregnancy and lactation due to lack of safety data. Common side effects include dehydration, diarrhea, gastrointestinal upset, muscle cramps, and water retention.
Rare serious effects have been reported in case studies (kidney inflammation, muscle breakdown, blood clots), though large studies haven't confirmed increased risk of liver injury. N-acetyl cysteine is generally well tolerated at typical doses; common oral side effects are diarrhea, dry mouth, heartburn, nausea, and vomiting.

N-acetyl cysteine also interacts moderately with blood pressure medications (antihypertensive drugs), blood thinners and antiplatelet drugs, chloroquine, and activated charcoal — potentially increasing low blood pressure risk, bleeding risk, reducing antimalarial effects, or interfering with charcoal absorption. Ribose can interact moderately with diabetes medications and insulin by potentially increasing the risk of low blood sugar (hypoglycemia).
Quercetin has moderate interactions with losartan, pravastatin, warfarin, quinolone antibiotics, cyclosporine, sulfasalazine, mitoxantrone, and other drugs that use the OATP transport system — risking increased drug levels, reduced antibiotic effectiveness, or increased bleeding.
