Gotu kola is generally well tolerated short-term, but quality evidence is limited. The most common side effects when taken by mouth are stomach irritation and nausea.

Topically, it may cause eczema or allergic contact dermatitis in some people. The main safety concern is liver toxicity: at least four case reports link gotu kola products to liver damage, though it's unclear if gotu kola itself was responsible or if contaminants in those products were to blame.

In one clinical trial where liver function was monitored, taking 120 mg daily for 6 months did not cause liver-function changes. Because there isn't enough safety data in pregnancy, it's traditionally been avoided.

It's also best avoided during breastfeeding, as safety data are unknown.
