Does it work?
Moderate evidence
The evidence is strongest for neem in gingivitis — research suggests it may help reduce gum inflammation and bleeding. There's also possibly effective data for lice and dental plaque.
For acne and a parasitic condition called tungiasis, we hold insufficient reliable evidence to say whether it works.
How safe is it?
Well-documented data
Neem leaf preparations appear to be well tolerated by adults when taken orally in the short term, though rigorous safety testing hasn't been done. Topical neem is generally well tolerated too, though contact dermatitis can occur in sensitive people.
The safety picture changes with neem oil and seed extracts — those can be toxic if swallowed, and neem oil in children has caused serious effects including metabolic acidosis and anemia. Rare but serious adverse effects reported with neem leaf include heart rhythm problems and kidney damage in adults, and hemolytic anemia in people with G6PD deficiency (a genetic blood condition).
Neem should be avoided during pregnancy because it may affect fertility and the developing baby. It's also best avoided while breastfeeding due to insufficient safety data.

The main concern is a group of drugs your liver breaks down using an enzyme called CYP3A4 — neem may slow that breakdown and raise drug levels in your blood. Lab studies suggest neem can also affect other liver enzymes (CYP1A2, CYP2C8, and CYP2C9) and a protein that moves drugs out of your cells (P-glycoprotein), though these interactions haven't been reported in people yet.

Neem may increase your low blood sugar (hypoglycemia) risk if you're on diabetes medication — some research shows neem can lower blood glucose on its own, and that effect could add to your drug's. It may also interfere with immunosuppressant medications (drugs given after transplant or for certain autoimmune conditions) by counteracting their effects through its own immune-stimulating properties.

Antidiabetes Drugs
Neem might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Some clinical research shows that neem can lower blood glucose levels in adults with type 2 diabetes, including those already taking metformin.
