Nexabiotic contains 24 active ingredients: inulin (a type of fiber) plus 23 probiotic bacterial and yeast strains. The probiotics include multiple Lactobacillus species (acidophilus, helveticus, gasseri, delbrueckii, and others), several Bifidobacterium species (longum, bifidum, breve), Bacillus strains (Bacillus subtilis via DE111 and Bacillus coagulans), Saccharomyces boulardii (a beneficial yeast), and Streptococcus thermophilus.
These live microorganisms are intended to support gut health. The capsules also contain rice dextrin and medium-chain triglycerides as inactive ingredients, and are formulated with delayed-release technology to help the organisms survive stomach acid.
Does it work?
Moderate evidence
The evidence for Nexabiotic's individual ingredients is mixed. Inulin is possibly effective for obesity, constipation, and diabetes, though evidence for its use in nonalcoholic fatty liver disease and iron deficiency anemia is insufficient.
Among the probiotics, Saccharomyces boulardii is possibly effective for several types of diarrhea (rotaviral, travelers', antibiotic-associated, and Clostridioides difficile infection) as well as Helicobacter pylori infection. Lactobacillus acidophilus is possibly effective for irritable bowel syndrome, Helicobacter pylori, antibiotic-associated diarrhea, and bacterial vaginosis.
Other strains show possible effectiveness for specific conditions—Bifidobacterium bifidum for irritable bowel syndrome and respiratory tract infections, Bacillus coagulans for constipation and irritable bowel syndrome—but many of the included strains lack sufficient evidence to rate their effectiveness for the conditions this product may target.
How safe is it?
Well-documented data
Inulin is likely safe in pregnancy and lactation but commonly causes gas, bloating, diarrhea, constipation, and gastrointestinal cramps, especially at higher doses. Most probiotic strains in this product are generally well tolerated in healthy adults and children, though they pose a rare risk of serious infection (bacteremia or sepsis) in critically ill or severely immunocompromised patients, particularly those with central venous catheters or very weak immune systems.
Saccharomyces boulardii rarely causes fungemia in critically ill patients and occasionally causes abdominal cramps, flatulence, nausea, or vomiting. For pregnancy and lactation, limited safety data exist for most of these strains—Lactobacillus acidophilus and Bifidobacterium longum are rated possibly safe, Streptococcus thermophilus is possibly safe in pregnancy, but data for lactation and other strains are insufficient or not on file.

Before taking Nexabiotic, double-check with your doctor or pharmacist if you take antibiotics—the live organisms in this product may lose effectiveness if taken at the same time, so spacing doses apart is important. If you take antifungal drugs, the same concern applies to Saccharomyces boulardii (a live yeast in the blend).
If you take diabetes medications, watch for signs of low blood sugar (hypoglycemia), as inulin may theoretically increase this risk. No interactions are documented for several of the probiotic strains we could not check, so be sure your pharmacist reviews your complete medication list.

Bifidobacterium longum

Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.

Lactobacillus delbrueckii LE

Theoretically, taking Lactobacillus delbrueckii with antibiotic drugs might decrease the effectiveness of L. delbrueckii.

Inulin
Antidiabetes Drugs
Theoretically, inulin might increase the risk of hypoglycemia with antidiabetes drugs.
Some clinical research shows that inulin improves glycemic control in patients with diabetes; however, it is unclear if it has hypoglycemic effects.
Saccharomyces boulardii
Antifungals
Theoretically, taking antifungals with Saccharomyces boulardii might decrease the effectiveness of Saccharomyces boulardii.
S. boulardii is a live yeast. Therefore, simultaneously taking antifungals might kill a significant number of the organisms.
