Boswellia serrata is generally well tolerated when taken by mouth short-term, though quality and strength vary widely between brands. The most common side effects are abdominal pain, diarrhea, headache, heartburn, itching, and nausea.

Dizziness and vertigo have been reported in some trial participants. A rare serious side effect—bezoar formation (a mass in the stomach)—has occurred with very large amounts of gum resin.

However, other in vitro research suggests that, when coupled with calcium ions, boswellic acids containing the keto group have immunostimulant properties within specific cell signaling pathways.
