This supplement contains 3 active ingredients: buchu leaf, saw palmetto berry, and uva-ursi (bearberry) leaf. Buchu is traditionally used as a diuretic and urinary support agent.
Saw palmetto is often taken for prostate health and urinary symptoms. Uva-ursi is also used for urinary tract support and has been employed short-term for urinary complaints.
The product also contains gelatin and water as inactive ingredients.
Does it work?
Not established
The evidence for these ingredients is mixed. Saw palmetto is rated possibly ineffective for benign prostate enlargement (BPH) — the condition many men take it for — though it's rated possibly effective specifically for transurethral resection of the prostate (TURP), a surgical procedure.
For other uses like androgenic alopecia (male-pattern hair loss), chronic bronchitis, and lower urinary tract symptoms, there is insufficient reliable evidence to rate saw palmetto's effectiveness. Uva-ursi is rated possibly ineffective for urinary tract infections and has insufficient evidence for bronchitis and BPH.

How safe is it?
Well-documented data
Buchu is generally used in food amounts, but medicinal doses lack strong safety data and the essential oil may be irritating. Saw palmetto is generally well tolerated in adults; the most common side effects are mild and infrequent — abdominal pain, constipation, diarrhea, dizziness, headache, and nausea.
Rare cases of heart rhythm problems and blood pressure changes have been reported. Uva-ursi is generally well tolerated at low doses for short-term use, but can be toxic at high doses or with prolonged use.
Common side effects are diarrhea, nausea, stomach upset, and vomiting. At very high doses (20 grams or more of dried herb), serious effects like convulsions, delirium, and breathing problems have been reported.
Regarding pregnancy and breastfeeding: buchu is likely unsafe in pregnancy (it's traditionally thought to affect the uterus) and possibly safe while breastfeeding. Saw palmetto is likely unsafe during both pregnancy and breastfeeding because of possible hormonal effects and lack of safety data.
Uva-ursi is likely unsafe in pregnancy because it may stimulate the uterus; lactation safety information is not on file. Talk with your doctor or pharmacist before taking this product if you are pregnant, planning to become pregnant, or breastfeeding.

Before taking this product, check with your doctor or pharmacist if you use blood thinners or antiplatelet drugs (including aspirin, warfarin, clopidogrel, enoxaparin, and others) — both buchu and saw palmetto may increase bleeding risk. If you take lithium, both buchu and uva-ursi may raise its levels.
If you're on estrogens or hormonal contraceptives, saw palmetto may reduce their effectiveness. If you take medications broken down by your liver through CYP2C19 or CYP3A4 pathways (a broad group that includes many common drugs), or if you're on drugs that acidify your urine, uva-ursi's effects may be altered.

Prostate Support by TerraVita Premium Collection contains three active ingredients — buchu, saw palmetto, and uva-ursi — and together they interact with a substantial number of medications.
The most serious interactions are Moderate in severity. Buchu and saw palmetto both may increase bleeding risk with blood thinners (anticoagulants and antiplatelet drugs) like warfarin, aspirin, and clopidogrel.
Buchu also carries concern for liver damage when combined with hepatotoxic drugs, especially at higher doses.

Saw palmetto may reduce how well estrogens and hormonal contraceptives work, potentially lowering their effectiveness. Uva-ursi interacts with several drug-metabolizing pathways: it may slow how your body breaks down medications processed through CYP2C19 and CYP3A4 enzymes, and theoretically could affect drugs eliminated through glucuronidation.
Both buchu and uva-ursi may raise lithium levels, which could require a dose adjustment.
Uva-ursi may also reduce its own effects if you're taking medications that acidify your urine, and it may alter levels of drugs transported by a protein called P-glycoprotein (a Minor interaction). Altogether, these interactions span 1,044 individual medications.

Uva-Ursi
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, uva ursi may decrease the metabolism of CYP2C19 substrates.
In vitro, uva ursi appears to inhibit cytochrome CYP2C19. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.
Glucuronidated Drugs
Theoretically, uva ursi may increase levels of drugs metabolized by glucuronidation.
In vitro, uva ursi extract appears to strongly inhibit UDP-glucuronosyltransferase (UGT) 1A1 (UGT1A1). However, uva ursi extract does not appear to inhibit UGT1A1 in animal models. This effect has not been reported in humans.
Lithium
Theoretically, uva ursi may increase lithium levels, necessitating a decrease in dose.
Uva ursi may have diuretic properties. Diuretics may increase lithium reabsorption with sodium in the proximal tubule of the kidney. Theoretically, uva ursi might reduce excretion and increase levels of lithium.
Urinary Acidifying Agents
Effects of uva ursi in the urinary tract may be reduced by urinary acidifying agents.
Uva ursi seems to work best in alkaline urine. Theoretically, taking uva ursi with medications known to acidify the urine may decrease any effects of uva ursi on the urinary tract.
P-Glycoprotein Substrates
Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.
Buchu
Anticoagulant/Antiplatelet Drugs
Buchu may have antiplatelet effects. Theoretically, buchu may enhance the effects of anticoagulant or antiplatelet drugs and increase the risk of bleeding in some patients. Some anticoagulant or antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.
Hepatotoxic Drugs
Buchu contains pulegone, a known hepatotoxin. There is some concern that buchu may adversely affect the liver, especially when the leaf is used in large doses or the oil is ingested. Theoretically, concomitant use with hepatotoxic drugs might increase the risk of liver damage. Some of these drugs include acarbose (Precose, Prandase), amiodarone (Cordarone), atorvastatin (Lipitor), azathioprine (Imuran), carbamazepine (Tegretol), cerivastatin (Baycol), diclofenac (Voltaren), felbamate (Felbatol), fenofibrate (Tricor), fluvastatin (Lescol), gemfibrozil (Lopid), isoniazid, itraconazole, (Sporanox), ketoconazole (Nizoral), leflunomide (Arava), lovastatin (Mevacor), methotrexate (Rheumatrex), nevirapine (Viramune), niacin, nitrofurantoin (Macrodantin), pioglitazone (Actos), pravastatin (Pravachol), pyrazinamide, rifampin (Rifadin), ritonavir (Norvir), rosiglitazone (Avandia), simvastatin (Zocor), tacrine (Cognex), tamoxifen, terbinafine (Lamisil), valproic acid, and zileuton (Zyflo)
Lithium
Buchu is thought to have diuretic properties. Theoretically, buchu might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
Saw Palmetto
Anticoagulant/Antiplatelet Drugs
Saw palmetto might increase the risk of bleeding with anticoagulant or antiplatelet drugs.
Saw palmetto is reported to prolong bleeding time. Theoretically, it might increase the risk of bleeding when used concomitantly with anticoagulant or antiplatelet drugs.
Contraceptive Drugs
Saw palmetto might reduce the effectiveness of contraceptive drugs.
Saw palmetto might have antiestrogenic effects. Theoretically, it might interfere with contraceptive drugs taken concomitantly.
Estrogens
Saw palmetto might reduce the effectiveness of estrogens.
Saw palmetto might have antiestrogenic effects. Theoretically, it might interfere with estrogens taken concomitantly.
