Rarely, raw shiitake can cause a skin eruption called shiitake dermatitis (resembling whiplash marks), sometimes with whole-body symptoms; large unchewed pieces have occasionally caused intestinal blockage.

Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
