Shiitake eaten as cooked food is generally well tolerated. At supplement doses, however, the safety data is limited, and caution is advised.
The most common side effects from oral use are abdominal discomfort, bloating, diarrhea, nausea, and vomiting. Rarely, raw shiitake can cause a skin eruption called shiitake dermatitis (resembling whiplash marks), sometimes with whole-body symptoms; large unchewed pieces have occasionally caused intestinal blockage.
People who handle or inhale shiitake extensively — such as growers — may develop allergic contact dermatitis, asthma, rhinitis, conjunctivitis, or even hypersensitivity pneumonitis (an inflammatory lung condition with breathing difficulty, chest pain, dry cough, and asthma).

Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Immunosuppressants
Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
In vitro evidence suggests that shiitake mushroom extracts stimulate immune function.
