Enasidenib isn't a prodrug, so more drug in your system would mean stronger effects and a greater chance of side effects like nausea, fatigue, or changes in your liver and blood test numbers.

Since Enasidenib is partly handled by that same enzyme, the worry is that adrafinil could nudge your body to clear it a little faster, which might slightly lower its levels and, in theory, weaken how well it fights the cancer cells.

Here's the simple version: Enasidenib (Idhifa) can block a liver enzyme called CYP3A4, and that same enzyme is part of how your body clears modafinil. Remember, adrafinil is a prodrug that your liver turns into modafinil, which is what actually keeps you awake and alert. If enasidenib slows that enzyme down, modafinil could linger longer and hit harder than expected.
In real life that might mean more trouble sleeping, jitteriness, a faster or pounding heartbeat, headache, or feeling anxious. Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
