Fluvoxamine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the levels and clinical effects of CYP1A2 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.

Here's the situation with these two. Your body relies on a liver enzyme called CYP1A2 to break down fluvoxamine. Adrafinil turns into modafinil in your body, and modafinil may gently speed up (induce) that same enzyme. In theory, that could clear fluvoxamine a little faster and make it less effective.

If that happened, you might notice your OCD or social anxiety symptoms creeping back, since fluvoxamine works by keeping serotonin levels steady. I want to be honest though: this is only a theoretical concern based on lab studies, and the effect looks weak. It hasn't been proven in real patients. Still, since adrafinil is unregulated and hard on the liver, please run it by your pharmacist or doctor first.

Here's the connection worth knowing. Once your liver converts adrafinil into modafinil, that modafinil is partly cleared by an enzyme called CYP3A4. Fluvoxamine (Luvox) can slow that enzyme down. In theory, this could let modafinil build up a bit higher than expected, meaning more of its stimulant-like effects.

What might that feel like? Possibly more trouble sleeping, jitteriness, a faster heartbeat, headache, or feeling on edge. I want to be honest though: this is a theoretical concern based on how the drugs work, not from strong real-world studies. Since adrafinil is unregulated and rough on the liver, please talk with your doctor or pharmacist before combining it with fluvoxamine.

What Is Fluvoxamine Used For?

Brand name: Luvox

Both fluvoxamine maleate tablets and Fluvoxamine Maleate extended-release capsules are approved to treat obsessive-compulsive disorder (OCD) — a condition marked by unwanted, recurring thoughts (obsessions) and/or repetitive behaviors (compulsions) that cause significant distress or interfere with daily life.

Fluvoxamine maleate tablets are approved for adults and for children and adolescents ages 8 to 17. Fluvoxamine Maleate extended-release capsules are approved for adults with OCD; their use in pediatric patients naive to fluvoxamine requires special prescriber consideration regarding the lowest available dose.
