Theoretically, adrafinil might increase the levels and clinical effects of CYP2C9 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to reduce CYP2C9 expression in vitro.

Here's the gist with this combo product. The ibuprofen portion is cleared by a liver enzyme called CYP2C9. Adrafinil turns into modafinil in your body, and lab studies suggest modafinil may dial down that enzyme a bit. In theory, that could let ibuprofen build up slightly, which might nudge up its usual side effects like stomach upset, heartburn, or, with regular use, kidney strain. The pseudoephedrine half isn't really part of this interaction.

I want to be honest though: this is theoretical, based on test-tube data, not real patient reports. For occasional use it's unlikely to cause trouble. Still, since adrafinil is unregulated and hard on the liver, please run it by your pharmacist or doctor before pairing them.

The piece of this combo product that matters here is the pseudoephedrine, which is a stimulant decongestant. Adrafinil turns into modafinil in your liver, and modafinil is a wakefulness drug with its own stimulant-like activity. Stack those two together and the concern is simply additive: more stimulation than either one alone.

In real life that could mean a faster or pounding heartbeat, higher blood pressure, jitteriness, headache, trouble sleeping, or feeling wired. The ibuprofen part isn't really the issue. Keep in mind this is a theoretical interaction based on how these drugs work, not strong clinical proof, so it may be mild for many people. Still, if you have heart or blood pressure issues, please check with your pharmacist or doctor before pairing them.
