Mifepristone falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Adrafinil interacts with.

Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the thing to know about pairing these two. Your liver uses an enzyme called CYP3A4 to turn adrafinil into modafinil, the active wakefulness drug. Mifepristone (Korlym) blocks that same enzyme. In theory, slowing this enzyme could let modafinil build up a bit more than usual, which might mean stronger stimulant effects like jitteriness, a racing heart, trouble sleeping, headache, or higher blood pressure.

I'll be honest with you though: this is a theoretical concern based on how the drugs work on paper, not on real patient reports. That said, adrafinil is unregulated and hard on the liver, and Korlym is a serious medication. Please don't start or stop either one on your own. Run it by your doctor or pharmacist first.

Here's the short version: your body turns adrafinil into modafinil, and modafinil can gently speed up a liver enzyme called CYP3A4. That same enzyme is one of the main pathways that breaks down mifepristone (Korlym). So in theory, pairing the two could clear mifepristone a bit faster and leave less of it working in your body.

For someone using Korlym to manage high blood sugar in Cushing's syndrome, weaker drug levels could mean it doesn't control cortisol as well, so blood sugar might creep back up. I'll be honest though: this concern is theoretical, based on how the drugs work rather than real cases. Mifepristone also stays in the body a long time. Please don't stop or change anything on your own. Just run it by your doctor or pharmacist first.

About Mifepristone (Korlym)

What Is Mifepristone (Korlym) Used For?

Brand names: Mifeprex, Korlym

Korlym is prescribed for adults with endogenous Cushing's syndrome — a condition where the body produces too much cortisol — who also have type 2 diabetes or trouble controlling blood sugar. It works to bring those blood sugar levels under control.

It is only for blood sugar problems that are directly caused by Cushing's syndrome. It should not be used to treat ordinary type 2 diabetes unrelated to this condition.
