Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the simple version. Nefazodone (Serzone) is an antidepressant that also acts as a fairly strong blocker of a liver enzyme called CYP3A4. That same enzyme is what clears modafinil, the active wakefulness-promoting compound your body makes from adrafinil. So if you take both, nefazodone could slow that cleanup and let modafinil build up higher than expected.

In real life that might mean more of modafinil's stimulant effects, like trouble sleeping, jitteriness, a racing or pounding heart, headache, or anxiety. Keep in mind this is a theoretical concern based on how the drugs work, not something proven in studies. Still, adrafinil is unregulated and rough on the liver, so please check with your pharmacist or doctor before combining them.

Here's the situation with these two. Your body turns adrafinil into modafinil, and modafinil can gently speed up a liver enzyme called CYP3A4. Nefazodone happens to be cleared partly by that same enzyme. So in theory, taking adrafinil could nudge your nefazodone levels a little lower, which might make it work less well for your depression over time.

I want to be honest though: this is a theoretical concern based on how the drugs behave, not something proven in studies. The effect of modafinil on this enzyme is described as weak. Nefazodone isn't a prodrug, so the direction here is reduced effect, not buildup. Still, adrafinil is unregulated and rough on the liver, so please loop in your doctor or pharmacist before combining them.

About Nefazodone

What Is Nefazodone Used For?

Brand name: Serzone

Nefazodone Hydrochloride tablets are approved to treat depression, specifically major depressive disorder (MDD). Clinical trials established its effectiveness over 6 to 8 weeks in outpatients and 6 weeks in hospitalized patients with depression.

Nefazodone has also been shown to help prevent depression from coming back in patients who responded well to initial treatment. Because of a serious risk of liver failure, prescribers are advised to consider other antidepressants before turning to nefazodone.
