Here's the gist of this one. Your leukemia medicine, nilotinib (Tasigna), can gently slow down a liver enzyme called CYP3A4. That same enzyme is the one that turns adrafinil into its active form, modafinil. So in theory, taking them together could let modafinil build up a bit more than usual, which may mean stronger stimulant-type effects like trouble sleeping, jitteriness, a faster heartbeat, headache, or feeling anxious.

I want to be honest that this is a theoretical concern based on how the drugs work, not something proven in real people, so I'm not trying to alarm you. The bigger issue is that adrafinil is unregulated and can stress the liver, while nilotinib already needs careful monitoring. Please check with your oncology team or pharmacist before mixing the two.

Here's the gist for nilotinib (Tasigna). Your liver clears this leukemia medicine using an enzyme called CYP3A4. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that same enzyme. In theory, that could push your liver to break down nilotinib a little faster, leaving less of it in your blood.

Why that matters: nilotinib needs to stay at the right level to keep your CML in check, so lower levels could mean it works less well over time. To be clear, this concern is theoretical and based on how the drugs behave, not on strong studies. Still, given how important this cancer drug is, please don't add adrafinil without first checking with your oncologist or pharmacist.

About Nilotinib

What Is Nilotinib Used For?

Brand name: Tasigna

  • Nilotinib is used to treat certain types of chronic myeloid leukemia (CML; a type of cancer of the white blood cells) who have recently found to have this condition in adults and children 1 year of age and older. It is also used to treat certain types of CML in adults whose disease could not be treated successfully with imatinib (Gleevec) or adults who cannot take imatinib because of side effects. Nilotinib is also used to treat certain types of CML in children 1 year of age or older whose disease could not be treated successfully with other tyrosine kinase inhibitor therapies or who cannot take these medications because of side effects. Nilotinib is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps to stop or slow the spread of cancer cells.
