Theoretically, drugs that inhibit CYP3A4 might increase the levels and clinical effects of adrafinil.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil is metabolized by CYP3A4.

Here's the thing with Paxlovid: the ritonavir part is a strong blocker of a liver enzyme called CYP3A4. That same enzyme helps clear modafinil, which is what your body makes after taking adrafinil. So while you're on Paxlovid, the modafinil from your adrafinil could hang around longer and build up to higher levels than usual.

In practice, that might mean more of modafinil's stimulant effects, things like trouble sleeping, jitteriness, headache, a faster or pounding heartbeat, anxiety, or higher blood pressure. This is a theoretical concern based on how the drugs work, not something proven in studies, so I don't want to alarm you. Paxlovid is also only a 5-day course. Still, please check with your pharmacist or doctor before combining them, especially since adrafinil can stress the liver.

Here's the gist with Paxlovid (nirmatrelvir plus ritonavir). When your liver turns adrafinil into modafinil, that modafinil can mildly speed up an enzyme called CYP3A4. Since nirmatrelvir is partly cleared through that same enzyme, the worry is that adrafinil could nudge your body to break it down a little faster, which might slightly lower its levels and weaken how well it fights the COVID virus.

I'll be honest though: this is a theoretical concern based on how the drugs work, not proven harm, and modafinil is only a weak inducer. Paxlovid is also a short 5-day course. Still, ritonavir itself interacts with a long list of things, so please loop in your pharmacist or doctor before combining them.
