Here the concern flows in one direction, and it's the opposite of what people often assume. Your cancer medicine, osimertinib (Tagrisso), is a mild blocker of a liver enzyme called CYP3A4. Adrafinil leans on that very same enzyme to turn into its active form, modafinil. So in theory, taking osimertinib alongside adrafinil could slow adrafinil's breakdown a bit and let more modafinil build up, which may mean stronger stimulant effects like jitteriness, a racing heart, trouble sleeping, headache, or higher blood pressure.

I want to be honest that this is a theoretical interaction based on how the drugs work, not real-world reports. Still, adrafinil is unregulated and rough on the liver, and you're already on a serious cancer drug. Please loop in your oncology team or pharmacist before combining them.

Here's the concern with pairing these two. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up a liver enzyme called CYP3A4. Your cancer medicine, Tagrisso (osimertinib), is partly cleared by that same enzyme. So in theory, taking adrafinil could nudge your body into clearing osimertinib a little faster, which might lower its blood levels and make it work less well against your lung cancer.

I want to be honest about the evidence here: this is theoretical, based on how the drugs behave, not on real patient cases. But with a cancer medicine you don't want to gamble on weaker effect. About Osimertinib

What Is Osimertinib Used For?

Brand name: Tagrisso

Tagrisso is used to treat adults with non-small cell lung cancer (NSCLC) whose tumors have specific EGFR mutations — either an exon 19 deletion or an exon 21 L858R mutation — confirmed by an FDA-approved test. It is used in several settings: after surgery to reduce the risk of cancer returning (adjuvant therapy), for locally advanced stage III NSCLC that has not progressed after chemoradiation, and as a first-line treatment for locally advanced or metastatic NSCLC (including in combination with pemetrexed and platinum-based chemotherapy).

Tagrisso is also approved for adults with metastatic NSCLC that carries a specific resistance mutation called T790M, when the cancer has stopped responding to another EGFR-targeted therapy.
