Pomalidomide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the levels and clinical effects of CYP1A2 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP1A2 activity in vitro.

Here's the idea behind this pairing. Once your liver turns adrafinil into modafinil, that modafinil may slightly speed up an enzyme called CYP1A2. Since pomalidomide (Pomalyst) is partly broken down by that same enzyme, a faster enzyme could clear the drug a little quicker, which in theory might lower its levels and make it work less well against your multiple myeloma or Kaposi's sarcoma.

I want to be honest about how soft this is. It is only a theoretical concern from lab data, and modafinil is a weak inducer at most, so a real-world drop in benefit is far from certain. That said, pomalidomide is a serious cancer medicine you don't want to undertreat. Please don't add adrafinil without first running it by your oncologist or pharmacist.

Here's the gist for Pomalyst (pomalidomide). Your body clears this cancer medicine partly through a liver enzyme called CYP3A4. Adrafinil turns into modafinil in your liver, and modafinil can mildly speed up that same enzyme. On paper, that could push pomalidomide out of your system a little faster, which might mean less of the drug working against your myeloma or Kaposi's sarcoma.

I want to keep this in perspective. This concern is theoretical, based on how the drugs behave rather than real patient reports, and the enzyme effect is considered weak. Still, pomalidomide is serious cancer therapy where you don't want its effect quietly weakened. Please don't change anything on your own. Loop in your oncology team or pharmacist before combining these.

It is sometimes sold as a nootropic supplement, but it is unregulated, carries real risks (especially to the liver), and is banned in some sports and countries. About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
