Ranolazine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist for these two together. Your body turns adrafinil into modafinil, and modafinil can gently speed up a liver enzyme called CYP3A4. Ranolazine happens to be cleared by that very enzyme, so in theory a faster-working enzyme could lower the amount of ranolazine in your blood. That might mean your angina medicine works a little less well, leaving you with more chest pain or pressure than usual.

I want to be honest about the strength of this: it's based on how the drugs work on paper, not on solid studies, and modafinil is only a weak nudge to that enzyme. Still, since ranolazine is keeping your heart comfortable, please loop in your doctor or pharmacist before pairing it with adrafinil.

What Is Ranolazine Used For?

Brand names: Ranexa, Aspruzyo

Ranolazine treats chronic angina — the recurring chest pain or pressure that happens when the heart isn't getting enough blood and oxygen. It is used when other angina medicines aren't enough on their own.

Both the extended-release tablet (sold as Ranexa and generic ranolazine) and the oral powder form (sold as Aspruzyo Sprinkle) are approved for this use. Ranolazine can be combined with other heart medicines such as beta-blockers, nitrates, calcium channel blockers, anti-platelet drugs, statins, ACE inhibitors, and angiotensin receptor blockers.
