Why this applies to Vorapaxar
Vorapaxar falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with these two. Once your liver turns adrafinil into modafinil, that modafinil can gently speed up an enzyme called CYP3A4. Your blood thinner vorapaxar (Zontivity) happens to be cleared by that same enzyme, so in theory revving it up could lower vorapaxar levels in your body and make it work a little less well.
Since vorapaxar's whole job is to keep platelets from clumping and forming dangerous clots, weaker levels could mean less protection against heart attack or stroke, which is exactly why you're taking it. That said, this is only a theoretical concern based on how the drugs behave, and modafinil is a weak inducer. Please don't change anything on your own. Just loop in your doctor or pharmacist before adding adrafinil.

What Is Vorapaxar Used For?
Brand name: Zontivity
Zontivity (vorapaxar) is used to lower the risk of heart attack, stroke, cardiovascular death, and the need for urgent procedures to open blocked arteries. It is prescribed for people who have already had a heart attack or who have peripheral arterial disease (PAD) — a condition where arteries outside the heart become narrowed or blocked.
Zontivity is not used alone. It is given alongside aspirin and/or clopidogrel as part of a broader antiplatelet treatment plan.
