Here's the gist for your antifungal. Your liver clears voriconazole partly through an enzyme called CYP2C19. Once your body turns adrafinil into modafinil, that modafinil may slow this same enzyme down a bit. The result, at least in theory, is that voriconazole could clear more slowly and build up higher than expected in your bloodstream.

Higher voriconazole levels can mean more side effects, like visual changes (blurry vision or seeing odd light flashes), nausea, headache, or liver strain. I'll be honest though: this concern is theoretical, based on lab studies, not real patient cases. Still, adrafinil is unregulated and hard on the liver, and voriconazole is already tough on the liver. Please run this combo by your doctor or pharmacist before mixing them.

Here's the connection worth knowing about. Once your liver turns adrafinil into modafinil, that modafinil is cleared mainly by a liver enzyme called CYP3A4. Voriconazole is a strong blocker of that same enzyme. So if you take the two together, the modafinil in your system may not break down as quickly and could build up higher than expected. In real terms, that might mean more jitteriness, a racing or pounding heart, trouble sleeping, headache, or anxiety.

I'll be honest with you: this is a theoretical concern based on how these drugs are processed, not something proven in studies, so I don't want to overstate it. Here's the idea behind this pairing. Your body converts adrafinil into modafinil, and lab studies hint that modafinil may slow down an enzyme called CYP2C9. Since voriconazole is partly cleared by that same enzyme, blocking it could let voriconazole build up a bit higher than usual in your system. More drug around can mean more side effects, things like visual changes (blurry vision or seeing flashes of light), nausea, headache, or in some cases liver irritation.

That said, this is a theoretical concern based only on test-tube data, not real patient reports, so I wouldn't panic. One thing worth flagging: both adrafinil and voriconazole can stress the liver, so combining them isn't ideal. Please run this by your doctor or pharmacist before mixing the two.

Here's the concern with pairing these two. Once your liver turns adrafinil into modafinil, that modafinil can mildly speed up an enzyme called CYP3A4. Voriconazole happens to be cleared in part by that same enzyme, so in theory your body could break it down a little faster and leave less of it in your blood.

Why that matters: voriconazole is fighting a serious fungal infection, and lower levels could mean it works less well. I want to be honest though, this is a theoretical interaction based on how the drugs behave, not something proven in studies, so it may not amount to much. Still, because the infections voriconazole treats are dangerous, I'd run any adrafinil use past your doctor or pharmacist first.

About Voriconazole

What Is Voriconazole Used For?

Brand name: Vfend

Voriconazole treats several serious fungal infections. Across all available forms — tablets, oral suspension, and intravenous (IV) injection — it is used for invasive aspergillosis (a dangerous mold infection), candidemia (Candida fungus in the bloodstream) in patients who are not neutropenic (not severely low on white blood cells), other deep-tissue Candida infections, and serious infections caused by Scedosporium apiospermum or Fusarium species in patients who cannot tolerate or have not responded to other antifungal treatments.

Esophageal candidiasis (a Candida infection of the esophagus, the swallowing tube) is also an approved use for most formulations. Note on the oral tablet form (generic voriconazole tablets): that specific tablet formulation is approved for patients aged 12 to 14 years weighing at least 50 kg, and all patients 15 years and older — not for younger or smaller children. The oral suspension and the IV injection are approved for patients 2 years and older for most of these conditions.
