These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Ashwagandha interacts with whole categories of medication. Acetaminophen, Caffeine, Isometheptene falls into 3 of those categories, so it can interact with Ashwagandha in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
HEPATOTOXIC DRUGS
1 of 3Why this applies to Acetaminophen, Caffeine, Isometheptene
Acetaminophen, Caffeine, Isometheptene falls under HEPATOTOXIC DRUGS — the group Ashwagandha interacts with.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Migralam is a combination headache product, and the piece that matters here is the acetaminophen (Tylenol). Acetaminophen is processed by your liver, and in higher doses or when other liver stressors are present, it can be hard on liver cells. Ashwagandha has been tied to rare reports of liver trouble too, so the concern is simply that two things that can tax the liver may add up.
To be clear, this is a theoretical caution based on how these substances behave, not proof that combining them will harm you. Still, watch for warning signs like unusual tiredness, nausea, belly pain, dark urine, or yellowing of your skin or eyes. Stick to the recommended acetaminophen dose, go easy on alcohol, and check with your pharmacist or doctor before pairing these.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
2 of 3Acetaminophen, Caffeine, Isometheptene also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the quick version for Migralam, which combines acetaminophen, caffeine, and isometheptene. The piece that matters here is the caffeine. Caffeine is cleared by a liver enzyme called CYP1A2, and lab studies hint that ashwagandha may speed that enzyme up. If it does, your body could clear the caffeine a little faster, meaning you might notice slightly less of that gentle pick-me-up and alertness that caffeine adds to the formula.
I want to be honest about how thin this evidence is. It comes only from test-tube and animal research, the severity is rated minor, and many people won't notice anything at all. Still, if you take ashwagandha regularly and want peace of mind, it's worth a quick chat with your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Acetaminophen, Caffeine, Isometheptene also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Migralam (acetaminophen, caffeine, and isometheptene) and ashwagandha. Some lab research hints that ashwagandha may speed up a liver enzyme called CYP3A4, which helps process certain medications. If that happens in the body, it could in theory clear parts of this combo a little faster, possibly making your migraine relief feel slightly weaker.
The good news? This is only a theoretical concern based on test-tube and animal studies, not real-world cases, and it's rated as minor. Acetaminophen mostly clears through other pathways, so any real effect is likely small. Still, if your headaches aren't responding the way they used to, mention the ashwagandha to your pharmacist or doctor so they can take a look.
