Why this applies to Alpelisib
Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Your body uses an enzyme called CYP3A4 to help break down Alpelisib (Piqray). Some lab research suggests ashwagandha might speed that enzyme up. If that were to happen in real life, your body could clear Alpelisib a little faster, which in theory might lower the drug's level and make it work less well against your cancer. Alpelisib is not a prodrug, so faster breakdown means less benefit, not more.
I want to be honest about the strength of this concern: it comes only from test-tube studies, the rating is minor, and we don't have human data showing it truly matters. Still, because Alpelisib treats a serious condition, please run any ashwagandha use past your oncologist or pharmacist first.

Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. What Is Alpelisib Used For?
Brand name: Piqray
Piqray is used with fulvestrant to treat adults with advanced or metastatic breast cancer that is hormone receptor (HR)-positive, HER2-negative and PIK3CA-mutated. It is for use after the cancer has progressed on or after an endocrine-based (hormone-blocking) treatment. An FDA-approved test must detect the mutation.
Vijoice is used for adults and children 2 years and older with severe PROS who need systemic (whole-body) therapy. This use has accelerated approval, which is based on response rate and duration of response. Continued approval may depend on confirmatory trials.
