Anagrelide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.

Here's the gist for these two. Your body uses an enzyme called CYP1A2 to help clear anagrelide, and lab studies hint that ashwagandha may speed that enzyme up a bit. If that happened in real life, your system could break down anagrelide a little faster, which might soften how well it keeps your platelet count down. For someone managing essential thrombocythemia or polycythemia vera, the worry would be platelets creeping back up.

That said, I want to keep this in perspective. This is based only on test-tube research, not on people, and it's rated as a minor interaction. So it's more of a heads-up than an alarm. Since anagrelide needs steady platelet monitoring anyway, just let your doctor or pharmacist know before adding ashwagandha so they can watch your bloodwork.

What Is Anagrelide Used For?

Brand name: Agrylin

Anagrelide — sold as Agrylin and as generic anagrelide capsules — is used to treat thrombocythemia secondary to myeloproliferative neoplasms. That means it's prescribed when a bone marrow disorder causes the body to produce too many platelets, raising the risk of dangerous blood clots.

By bringing platelet counts down to a safer range, anagrelide also helps reduce related symptoms, including bleeding and clotting events (thrombo-hemorrhagic events).
