Here's the simple version: ashwagandha can make you feel drowsy and calm on its own. Apadaz (benzhydrocodone with acetaminophen) is an opioid pain medicine, and opioids already cause sleepiness, slowed breathing, and a foggy head. Putting the two together could, in theory, stack those calming effects, so you might feel more sedated, dizzy, or out of it than you expect.
I want to be honest that this concern is theoretical, based on how ashwagandha behaves rather than strong human studies. Still, with a narcotic like this, I'd play it safe. Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.

Here's the concern with pairing ashwagandha and Apadaz: the acetaminophen part of this combo is processed by your liver, and in higher doses or with heavy use it can stress that organ. Ashwagandha has been tied (in rare reports) to liver problems of its own, like hepatitis and, very rarely, serious liver injury. Stacking two things that can tax the liver could, in theory, add up.
To be honest, this is a theoretical worry based on how these work, not proven head-to-head, so I don't want to alarm you. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, nausea, or unusual tiredness. Don't start or stop either one on your own. Here's the gist for Apadaz (benzhydrocodone with acetaminophen). There's a lab-based note suggesting ashwagandha may speed up an enzyme called CYP1A2, which could lower levels of medicines that enzyme handles. In theory, that might make a pain reliever feel a little weaker, so you could notice less relief from your dose.
I want to be honest with you: this is rated minor and comes only from test-tube research, not from people. The main pathways for benzhydrocodone don't lean heavily on CYP1A2, so I wouldn't expect a real-world problem for most folks. Here's something worth knowing about this pair. Benzhydrocodone is what we call a prodrug, meaning your body has to convert it into the active opioid (hydrocodone) before it can ease your pain. The lab research we have suggests ashwagandha may gently speed up a liver enzyme called CYP3A4, which is involved in handling this medication. In theory, that could mean a bit less pain relief from your Apadaz than you'd expect.
I want to be honest though: this is based only on test-tube and animal studies, the interaction is rated minor, and it's far from proven in real people. So I wouldn't lose sleep over it. Serotonergic Drugs

Here's the short version: opioids like the benzhydrocodone in Apadaz can nudge serotonin levels in the brain, and ashwagandha may gently tug on serotonin pathways too. On paper, stacking two things that touch serotonin raises a theoretical concern. But I want to be honest with you, this is a minor, low-evidence flag. The serotonin signal for ashwagandha comes mostly from animal studies, and there are no real case reports of problems when it's combined with serotonin-affecting medicines.
What I'd actually watch for is feeling extra sleepy, foggy, or dizzy, since both can be calming. Serotonin syndrome (agitation, sweating, fast heartbeat, shivering, muscle twitching) is unlikely here but worth knowing. Brand name: Apadaz
Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.
Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
