Brexpiprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.

Here's the gist with Rexulti (brexpiprazole) and ashwagandha. Your body clears brexpiprazole partly through a liver enzyme called CYP3A4. In lab studies, ashwagandha appears to speed that enzyme up. If that holds true in people, it could mean a bit more of the drug gets broken down, leaving slightly lower levels in your system.

In plain terms, that might make brexpiprazole work a little less well, so symptoms it helps control (mood, anxious or disordered thinking) could creep back. I want to be honest though: this is theoretical, based only on test-tube research, and rated minor. It may not matter in real life. Still, before adding ashwagandha, check with your pharmacist or prescriber so they can keep an eye on how you're doing.

Brexpiprazole also falls under Serotonergic Drugs.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. Here's the short version: this is a very mild, mostly theoretical concern. Brexpiprazole (Rexulti) gently nudges serotonin activity in the brain, and some animal research suggests ashwagandha may also tap into serotonin pathways. On paper, two things touching the same system could add up. In reality, there are no human case reports of problems when ashwagandha is paired with serotonin-affecting medications, so the risk looks low.

If you do take both, just stay aware of how you feel. Things like feeling unusually restless, jittery, drowsy, or having trouble sleeping are worth noting. None of this means you should stop either one. Before you add ashwagandha, run it by your pharmacist or doctor so they can look at your full medication list.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
