Buprenorphine/Naloxone falls under CNS DEPRESSANTS — the group Ashwagandha interacts with.
Ashwagandha seems to have sedative effects. Theoretically, this may potentiate the effects of barbiturates, other sedatives, and anxiolytics.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Suboxone and ashwagandha: the buprenorphine in it already has a calming, sedating side, and ashwagandha appears to be mildly relaxing on its own. Put them together and, in theory, you could feel a bit more drowsy, foggy, or sluggish than you would from your medication alone. I want to be upfront that this concern is theoretical, based on how each one behaves rather than on solid human studies, so for many people it may not cause any real trouble.
Still, with an opioid-based medicine like this, I'd rather you be cautious. If you notice extra sleepiness, dizziness, or slowed thinking, that's worth a call to your pharmacist or prescriber before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Suboxone fans: the buprenorphine part is partly handled by a liver enzyme called CYP3A4. Lab studies (not human studies) hint that ashwagandha may speed up that enzyme a little. If that holds true in people, your body could clear buprenorphine a bit faster, which in theory might make it feel slightly weaker. In someone who's stable on Suboxone, that could show up as mild withdrawal-type feelings like restlessness, achiness, or cravings creeping back.
That said, this is very weak, theoretical evidence rated as minor, so I wouldn't lose sleep over it. Don't change anything on your own. Just give your pharmacist or prescriber a heads-up before you start ashwagandha so they can keep an eye on how you're doing.
