Here's the gist with this pair. Your body uses a liver enzyme called CYP3A4 to help clear dextromethorphan, the active cough suppressant in Vicks Formula 44. Lab studies (done in test tubes, not in people) hint that ashwagandha might speed that enzyme up a bit. If that happened in real life, your body could break down the dextromethorphan a little faster, which might make your cough medicine wear off sooner or feel slightly less effective.

I want to be honest about the strength of this: it's only theoretical and based on early lab data, so it's rated minor. For an occasional cough syrup, it's very unlikely to cause a real problem. Still, if you take ashwagandha regularly and notice your cough relief isn't lasting, mention it to your pharmacist or doctor.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. Here's the gist with these two together. Dextromethorphan, the cough suppressant in products like Vicks Formula 44, has mild effects on serotonin in the brain. Ashwagandha may also nudge serotonin pathways, at least based on some animal research. In theory, stacking two things that touch serotonin could add up.

That said, this is a very low-level concern. There are no real-world reports of problems when ashwagandha is combined with serotonin-affecting drugs, and the evidence here is weak and mostly from lab animals. Serotonin syndrome (think agitation, fast heartbeat, sweating, shivering, or confusion) is unlikely with normal cough doses. Still, if you take other serotonin drugs too, it's worth a quick chat with your pharmacist before combining them.
