Doravirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.

Here's the gist for doravirine (Pifeltro): your body clears this HIV medicine using a liver enzyme called CYP3A4. Lab studies suggest ashwagandha may speed that enzyme up. If that happened in real life, your doravirine could be cleared a little faster, which in theory might lower its blood levels and make it work less well against HIV. With HIV medicines, keeping levels steady matters, since weaker control can let the virus rebound.

That said, this is theoretical and comes only from test-tube research, not from people, so we don't actually know it causes a problem. Still, because consistent HIV control is so important, I'd run any ashwagandha plan past your pharmacist or HIV provider first rather than starting it on your own.

About Doravirine

What Is Doravirine Used For?

Brand name: Pifeltro

Pifeltro (doravirine) is used together with other antiretroviral medications to treat HIV-1 infection in adults and in pediatric patients weighing at least 35 kg (about 77 lbs). It is approved for people who have never been on HIV treatment before, as well as for people who are already virologically suppressed — meaning their HIV is currently undetectable — and want to switch to a new stable regimen.

Pifeltro is not used alone; it must always be part of a combination HIV regimen. Patients switching to a Pifeltro-based regimen must have no history of treatment failure and no known resistance to doravirine.
