These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Ashwagandha interacts with whole categories of medication. Drospirenone, Ethinyl Estradiol falls into 3 of those categories, so it can interact with Ashwagandha in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
HEPATOTOXIC DRUGS
1 of 3Why this applies to Drospirenone, Ethinyl Estradiol
Drospirenone, Ethinyl Estradiol falls under HEPATOTOXIC DRUGS — the group Ashwagandha interacts with.
Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: your birth control pill (the drospirenone and ethinyl estradiol combo) is processed by your liver, and so is ashwagandha. On its own, ashwagandha has been tied to rare but real cases of liver trouble. The concern with combining them is that two things passing through the liver could, in theory, add stress to it. To be clear, this is a theoretical worry based on how these work, not something proven to happen often, so I don't want you to panic.
Still, it's worth watching for warning signs like yellowing of your skin or eyes, dark urine, belly pain on your right side, nausea, or unusual tiredness. If any of those show up, stop and call your doctor. Before adding ashwagandha, just check with your pharmacist or doctor first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
2 of 3Drospirenone, Ethinyl Estradiol also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist when you pair ashwagandha with your birth control pill (Yasmin, Gianvi, and similar). Lab studies hint that ashwagandha might speed up an enzyme called CYP1A2, which could, in theory, lower the levels of hormones like the ethinyl estradiol in your pill. If that happened, the pill might work a little less reliably, which for birth control means a small concern about breakthrough bleeding or reduced protection against pregnancy.
That said, I want to keep this in perspective. This is based only on test-tube research, not real people, and it's rated as a minor, theoretical interaction. So it's worth knowing about, but not worth panicking over. If you rely on this pill to prevent pregnancy, just mention the ashwagandha to your pharmacist or doctor so they can weigh in for your situation.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Drospirenone, Ethinyl Estradiol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your birth control pill (Yasmin, Gianvi, and similar) relies on steady hormone levels to do its job, and the body clears those hormones partly through a liver enzyme called CYP3A4. In lab and test-tube studies, ashwagandha seemed to speed that enzyme up a bit. If that happened in real life, it could nudge your hormone levels slightly lower and, in theory, make the pill a touch less reliable, which is the concern with any contraceptive.
Here's the honest part: this is only a theoretical, minor interaction based on lab work, not on real people, so it may not matter at all. Still, because an unplanned pregnancy is a big deal, I'd mention the ashwagandha to your pharmacist or doctor before relying on it long-term.
