Here is the interaction we found between these two, with the evidence behind it.
ANTIHYPERTENSIVE DRUGS
Why this applies to Fosinopril
Fosinopril falls under ANTIHYPERTENSIVE DRUGS — the group Ashwagandha interacts with.
Animal research suggests that ashwagandha might lower systolic and diastolic blood pressure. Theoretically, ashwagandha might have additive effects when used with antihypertensive drugs and increase the risk of hypotension.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: Fosinopril already works to bring your blood pressure down, and ashwagandha may gently nudge it lower too. Put them together and those effects could stack up. This isn't about how your liver breaks the drug down. It's simply two things pushing in the same direction.
If your pressure dips too far, you might feel lightheaded, dizzy, tired, or wobbly when standing up quickly. Worth knowing: this caution comes mostly from animal research, so it's more theoretical than proven in people. Still, if you already run on the lower side or take other blood pressure meds, mention it to your pharmacist or doctor before adding ashwagandha. Please don't stop or change your fosinopril on your own.

From our pharmacist’s overview on the Fosinopril page
“I would read the supplement list for fosinopril (Monopril), a blood pressure medicine, with genuine care, because a handful of these combinations can do real harm. Most of what interacts touches blood pressure itself, either adding to fosinopril's lowering effect or working against it, tyramine being a well-documented example of the la…”

Fosinopril Is Linked to Lower Levels of One Nutrient
Natural Medicines records Fosinopril as a medicine that can lower these. This is not an interaction with Ashwagandha — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
