Theoretically, taking ashwagandha with antihypertensive drugs might increase the risk of hypotension.
Animal research suggests that ashwagandha might lower systolic and diastolic blood pressure. Theoretically, ashwagandha might have additive effects when used with antihypertensive drugs and increase the risk of hypotension.

Opsynvi combines two blood pressure lowering medicines: macitentan and tadalafil. Both are used to widen blood vessels in the lungs for pulmonary hypertension, and together they can already nudge your blood pressure down. Ashwagandha may have a mild blood pressure lowering effect of its own, so stacking it on top could, in theory, push things a little too low.

If that happened, you might feel lightheaded, dizzy, or faint, especially when standing up quickly. I want to be honest though: this concern comes from animal research only, not human studies, so it is more of a caution than a proven problem. Please don't start or stop anything on your own. Just check with your pharmacist or doctor first, and mention any dizziness you notice.

Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.

Here's the concern with Opsynvi, which is a combo of macitentan and tadalafil. This isn't about ashwagandha changing how the drug is broken down. Instead, both ashwagandha and this medication can put stress on the liver on their own, so combining them may add up. Macitentan in particular carries warnings about liver enzyme changes, and ashwagandha has been tied to rare but real cases of liver injury.

To be fair, this is a theoretical worry based on how each affects the liver, not proven cases of the two together. Still, watch for signs like yellowing skin or eyes, dark urine, belly pain, unusual tiredness, or nausea. Please talk with your pharmacist or doctor before adding ashwagandha, since your liver may already be monitored on this drug.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.

Opsynvi combines two medicines, macitentan and tadalafil, that both open up blood vessels to treat pulmonary arterial hypertension. Both are handled in your body by a liver enzyme called CYP3A4. Some early lab research (not human studies) hints that ashwagandha might speed that enzyme up a little, which in theory could clear these drugs a bit faster and make them work slightly less well.

If that happened, you might notice your PAH feeling less controlled, such as more shortness of breath or fatigue. To be clear, this is only a theoretical concern from test-tube data, and it is rated minor, so real-world impact is likely small. Still, because keeping PAH steady matters, please check with your pharmacist or doctor before adding ashwagandha rather than changing anything on your own.
