Olanzapine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ashwagandha interacts with.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.

Here's the basic idea with these two. Your body uses an enzyme called CYP1A2 to break down olanzapine. Lab studies hint that ashwagandha may speed that enzyme up a bit, which in theory could clear olanzapine out of your system a little faster and make it slightly less effective. For olanzapine, weaker effect could mean your symptoms (like mood swings or disturbed thinking) aren't as well controlled.

That said, I don't want to overstate this. This concern comes only from test-tube research, not from people, so we really don't know if it matters in real life. Olanzapine isn't a prodrug, so the direction makes sense, but the signal is weak. Before adding ashwagandha, just run it by your pharmacist or doctor so they can keep an eye on how you're doing.

Olanzapine also falls under Serotonergic Drugs.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. Here's the gist with olanzapine (Zyprexa) and ashwagandha. Olanzapine nudges serotonin and dopamine signaling in the brain, and some animal research hints that ashwagandha may gently boost serotonin activity too. On paper, stacking two things that touch the same pathway could add up. In practice, though, this is rated a minor concern, and there are no real-world case reports of problems like serotonin syndrome when ashwagandha is paired with serotonin-affecting medicines.

So I wouldn't lose sleep over it, but I'd stay aware. If you ever notice things like unusual restlessness, agitation, shivering, a racing heart, or feeling more sedated than normal, check in with your pharmacist or doctor. And please loop them in before starting ashwagandha rather than changing your olanzapine on your own.

About Olanzapine

What Is Olanzapine Used For?

Brand name: Zyprexa

Olanzapine treats schizophrenia and the manic or mixed episodes of bipolar I disorder in both adults and adolescents (ages 13–17). It can also be used alongside valproate or lithium to treat manic or mixed episodes in adults. When combined with fluoxetine, olanzapine tablets and orally disintegrating tablets are used to treat depressive episodes associated with bipolar I disorder and treatment-resistant depression in adults.

Olanzapine intramuscular injection (short-acting) is used by healthcare providers to quickly calm acute agitation in adults with schizophrenia or bipolar I mania. Zyprexa Relprevv — a separate, long-acting intramuscular formulation available only through a restricted program — is indicated specifically for the treatment of schizophrenia in adults.
