Ashwagandha and Rizatriptan Benzoate

Here is the interaction we found between these two, with the evidence behind it.

Minor

Serotonergic Drugs

Why this applies to Rizatriptan Benzoate

Rizatriptan Benzoate falls under Serotonergic Drugs — the group Ashwagandha interacts with.

Some animal studies have reported that ashwagandha can enhance serotonergic transmission by altering certain serotonin (5-HT) receptors. However, there is no evidence to suggest that ashwagandha increases the risk of serotonin-related effects, and there have been no published case reports of serotonin syndrome when combined with other serotonergic drugs. Nevertheless, due to the lack of extensive studies on the matter and the fact that ashwagandha appears to affect serotonergic pathways, it would be prudent to exercise caution when combining it with drugs that affect serotonin. Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Maxalt (rizatriptan) and ashwagandha. Rizatriptan works on serotonin receptors in your brain to ease a migraine. Some animal research hints that ashwagandha may also nudge the serotonin system a little. On paper, stacking two things that touch serotonin raises a theoretical question about too much serotonin activity.

That said, this is a minor, low-evidence concern. There are no real-world reports of people running into trouble combining these two, and the data comes from animal studies, not human cases. Still, it's worth knowing the rare signs of too much serotonin: agitation, fast heartbeat, sweating, shivering, or muscle twitching. If you take both, mention it to your pharmacist or doctor and watch for those symptoms.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

Rizatriptan treats migraine headaches — with or without aura — when they occur. It is approved for adults and for children and teens ages 6 to 17. It is available as a regular tablet (Maxalt, Rizatriptan, Rizatriptan Benzoate), an orally disintegrating tablet (Maxalt, Maxalt-MLT, Rizatriptan Benzoate ODT), and a dissolving oral film placed on the tongue (Rizafilm).

Rizatriptan is for stopping a migraine once it starts — it is not meant to prevent migraines from happening, and it is not approved for cluster headaches. It should only be used after a doctor has confirmed a migraine diagnosis.
