Here's the concern with pairing these two: Tecvayli (teclistamab) can stress the liver on its own, and ashwagandha has been tied to rare but real cases of liver injury, including hepatitis and even liver failure. Stacking two things that can each be hard on the liver may, in theory, raise the chance of trouble. This isn't about ashwagandha changing how Tecvayli is broken down. It's simply about doubling up on potential liver strain.

Keep in mind this is a theoretical caution based on how each behaves, not proven cases of the two together. Still, watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and report them right away. Since you're already on a serious cancer therapy, please check with your oncology team or pharmacist before adding ashwagandha.

Ashwagandha has demonstrated immunostimulant effects in humans. Animal research has shown that ashwagandha can attenuate the immunosuppression caused by cyclophosphamide.

Here's the idea behind this pairing. Ashwagandha may give the immune system a little nudge to become more active. Teclistamab (Tecvayli) is a powerful immune-based therapy for multiple myeloma that works by directing your own T cells to attack cancer cells, so it deliberately revs up parts of your immune response. On paper, mixing an immune-stimulating herb with a treatment like this raises the question of whether ashwagandha could interfere with how the medicine is meant to behave.

That said, this is a theoretical concern only. The worry comes from lab and animal studies, not from people taking these two together, so we honestly don't know if it matters in real life. Because Teclistamab is such a serious, closely monitored therapy, please don't add ashwagandha (or stop anything) without first checking with your oncology team or pharmacist.

What Is Teclistamab-cqyv Injection Used For?

Brand name: Tecvayli

Tecvayli (teclistamab-cqyv) injection is used to treat adults with relapsed or refractory multiple myeloma — a cancer of plasma cells in the bone marrow — whose disease has come back or stopped responding to earlier treatments.

Tecvayli is approved in two settings: as monotherapy for patients who have received at least four prior lines of therapy (including a proteasome inhibitor, an immunomodulatory agent, and an anti-CD38 monoclonal antibody); and in combination with daratumumab and hyaluronidase-fihj for patients who have received at least one prior line of therapy including a proteasome inhibitor and an immunomodulatory agent.
