Ashwagandha and Valbenazine Tosylate

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Ashwagandha interacts with whole categories of medication. Valbenazine Tosylate falls into 2 of those categories, so it can interact with Ashwagandha in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CNS DEPRESSANTS

1 of 2

Why this applies to Valbenazine Tosylate

Valbenazine Tosylate falls under CNS DEPRESSANTS — the group Ashwagandha interacts with.

Theoretically, taking ashwagandha might increase the sedative effects of CNS depressants.
Ashwagandha seems to have sedative effects. Theoretically, this may potentiate the effects of barbiturates, other sedatives, and anxiolytics.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version for Ingrezza (valbenazine) and ashwagandha. Valbenazine already tends to make some people feel drowsy or tired. Ashwagandha is often taken for stress and sleep, and it can have its own mild calming, sedating effect. So the concern is really about adding two sleepy effects together, not about one changing how the other is broken down.

If you take both, you might notice more drowsiness, slowed thinking, or feeling a bit unsteady. I'll be honest though: this interaction is theoretical, based on how each one acts rather than on solid studies, so for many people it stays mild. Still, before adding ashwagandha, check with your pharmacist or doctor, especially if you drive or already feel sleepy on Ingrezza.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Valbenazine Tosylate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Ingrezza (valbenazine): your liver uses an enzyme called CYP3A4 to help process this medicine. In lab dish studies, ashwagandha appeared to speed up (induce) that same enzyme. If that happened in real life, your body might clear valbenazine a little faster, which could mean it works less well at calming the unwanted face, tongue, or body movements it's prescribed to treat.

I want to be honest about how soft this evidence is. This is only a theoretical concern from test-tube research, not from people, and it's rated minor. So I wouldn't panic. Still, because valbenazine is doing an important job, it's worth mentioning ashwagandha to your pharmacist or doctor before starting it, and let them know if your symptoms seem to creep back.
