Why this applies to Acetaminophen, Caffeine, Pyrilamine

Acetaminophen, Caffeine, Pyrilamine falls under GLUCURONIDATED DRUGS — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by uridine diphosphoglucuronosyl transferase (UGT). So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits UGT. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Some drugs that are metabolized through glucuronide conjugation include acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.

If you take a Midol-type product (acetaminophen, caffeine, and pyrilamine), here is what this is about. Your body clears acetaminophen partly by attaching a sugar molecule to it, a process called glucuronidation. In a lab dish, a compound found in breadfruit (artocarpin) slowed down that very pathway. In theory, that could let acetaminophen build up a bit and increase its effects, including, at higher amounts, stress on the liver.

That said, this has never been seen in real people, and the evidence is only from test-tube and animal work, so I would not lose sleep over it. Eating breadfruit as food is very unlikely to matter. Concentrated breadfruit leaf teas or extracts are the bigger unknown. If you use those regularly, just mention it to your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Caffeine, Pyrilamine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

If you enjoy breadfruit as a food, there's really nothing to worry about when you take a product like Midol Max Strength (acetaminophen, caffeine, and pyrilamine). The concern here is only theoretical and comes from lab and animal studies, where a breadfruit compound called artocarpin weakly slowed down a liver enzyme (CYP3A4). In plain terms, that enzyme helps clear certain medicines, so blocking it could let a drug build up a little more than usual.

For this specific combo, even if that mild slowdown happened, you might at most notice slightly stronger caffeine effects (jitteriness, a faster heartbeat, trouble sleeping) or a bit more drowsiness from the pyrilamine. The effect is rated minor and unproven in people. If you're using concentrated breadfruit leaf teas or extracts regularly, just check with your pharmacist first.
