Why this applies to Acetaminophen, Dextromethorphan, Pseudoephedrine

Acetaminophen, Dextromethorphan, Pseudoephedrine falls under GLUCURONIDATED DRUGS — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by uridine diphosphoglucuronosyl transferase (UGT). So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits UGT. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Some drugs that are metabolized through glucuronide conjugation include acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.

This combination cold and flu product contains acetaminophen (Tylenol), and that's the part of the mix this interaction is really about. Your body clears acetaminophen partly through a process called glucuronidation. A compound in breadfruit called artocarpin has been shown, in lab dishes only, to slow that pathway down. In theory, that could let acetaminophen build up a bit higher than usual.

Here's the honest part: this has never actually been seen in real people, and the evidence comes from test-tube studies, not humans. Eating breadfruit as a food is very unlikely to cause any problem. Concentrated leaf teas or extracts are the bigger question mark. Since too much acetaminophen can stress the liver, just keep your total daily dose in check and check with your pharmacist before using breadfruit medicinally.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Dextromethorphan, Pseudoephedrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the gist with this combo cold medicine and breadfruit. One ingredient, dextromethorphan (the cough suppressant), is partly broken down by a liver enzyme called CYP3A4. In lab dishes and animal studies, a compound in breadfruit appears to weakly slow that same enzyme down. In theory, that could let a little more dextromethorphan hang around, which might mean more drowsiness, dizziness, or a foggy feeling.

That said, this is a minor, mostly theoretical concern, and we have no human studies showing it actually matters. Eating breadfruit as food is very unlikely to cause a problem. Concentrated leaf teas or extracts are the bigger unknown. The acetaminophen and pseudoephedrine parts aren't really affected here. If you use breadfruit medicinally, just give your pharmacist a quick heads-up.
