Why this applies to Cabotegravir, Rilpivirine

Cabotegravir, Rilpivirine falls under GLUCURONIDATED DRUGS — the group Breadfruit interacts with.

Breadfruit might increase the levels of drugs metabolized by uridine diphosphoglucuronosyl transferase (UGT). So far, this interaction has not been reported in humans. However, in vitro evidence shows that the breadfruit constituent artocarpin inhibits UGT. Theoretically, this could decrease the clearance and increase levels of glucuronidated drugs. Some drugs that are metabolized through glucuronide conjugation include acetaminophen (Tylenol, others) and oxazepam (Serax), haloperidol (Haldol), lamotrigine (Lamictal), morphine (MS Contin, Roxanol), zidovudine (AZT, Retrovir), and others.

Here's the connection worth knowing. The cabotegravir piece of Cabenuva is partly cleared by a liver enzyme system called UGT (glucuronidation). A natural compound in breadfruit called artocarpin has been shown, in a test tube only, to slow that same enzyme down. In theory, if breadfruit blocked UGT, cabotegravir could hang around longer in your body, which might mean a bit more of its usual effects like injection-site soreness, headache, or feeling tired.

I want to keep this in perspective. This has never been seen in real people, and eating breadfruit as a normal food is very different from taking concentrated leaf teas or extracts. Since keeping HIV treatment steady really matters, just give your pharmacist or doctor a heads-up before using any breadfruit supplement product.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Cabotegravir, Rilpivirine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the good news on this one. The concern is purely theoretical. In lab and animal studies, a compound in breadfruit called artocarpin showed weak ability to slow down the CYP3A4 enzyme. Rilpivirine, one half of your Cabenuva shots, is partly handled by that same enzyme. If breadfruit blocked it in real life, rilpivirine levels could creep up a bit, which in theory might mean more side effects like headache, nausea, trouble sleeping, or mood changes.

That said, there's no human evidence this actually happens, and eating breadfruit as a food is very different from taking concentrated leaf teas or extracts. I wouldn't lose sleep over enjoying the fruit. If you're thinking about breadfruit leaf supplements for blood pressure or sugar, just run it by your HIV provider or pharmacist first.

What Is Cabotegravir and Rilpivirine Injections Used For?

Brand name: Cabenuva

CABENUVA (cabotegravir + rilpivirine) is used to treat HIV-1 infection in adults and adolescents 12 years and older who weigh at least 35 kg (about 77 lbs). It is not a starter treatment — it's designed for people who are already virologically suppressed, meaning their HIV is currently undetectable on a stable daily pill regimen and who have no history of treatment failure.

The goal is to keep the virus fully suppressed while freeing the person from taking a pill every day. Because it stays active in the body for weeks after each injection, strict adherence to the injection schedule is essential.
